After your appointment: Submit your application at medportal.omma.ok.gov. Upload your physician recommendation, Oklahoma proof of residency, proof of identity, and a digital photo. Pay the application fee with Visa, MasterCard, or Discover. The signed recommendation must be dated within 30 days of submission. Once approved, your physical card is mailed via USPS — your approval email or MedPortal status serves as valid patient ID at dispensaries while your card is in transit.
